Getting Your Practice Started

The QuitWorks At A Glance: Office Practice Guide walks you through the easy steps to identify smokers in your practice and enroll them in QuitWorks services. We also have several tools to help you get started.

What You Do in Five Easy Steps QuitWorks Takes It From There
1. Set up a system to identify tobacco use by patients when they arrive at the office practice. QW: Calls your patient - Upon receipt of an enrollment form, QuitWorks:
  • Conducts a 10-minute telephone interview to assess readiness to quit
  • Mails a customized Quit Kit
  • Offers multiple counseling options
  • Advises your patient on course of action
2. Talk with patients about tobacco use during the office visit. Give the patient a Think About It pamphlet at check-in. QW: Provides intensive counseling services, on the phone or through referral to in-person services
  • Five proactive telephone counseling sessions
  • Referral to more than 30 community tobacco treatment centers with trained counselors
3. Complete enrollment form, give patient QuitWorks Welcome Guide, and fax enrollment form to QuitWorks toll-free line. QW: Provides on-line help at http://www.trytostop.org
  • The QuitWizard, a self-directed counseling tool
  • Interactive bulletin board and user community
  • Expert advice and success stories
  • Access to a warehouse of materials to download to help the patient
4. Prescribe pharmacotherapy, if appropriate, for relief of withdrawal symptoms and to aid with stopping smoking. QW: Faxes report to referring provider several days after patient contact and indicates services selected.
5. Receive status reports, review, and file in patient medical record. QW: Sends a six-month patient quit status report to the provider identified on the enrollment form; aggregate reports may be customized for your hospital, if desired.


Tools for When You Start Using QuitWorks

A number of tools are available to help you get started. Brochures, pocket guides, buttons and posters can be ordered in bulk by calling 1-800-TRY-TO-STOP (1-800-879-8678) or download an order form online from this website. There is a small shipping charge, but the materials are free. Go to QuitWorks Tools for more information.

Tools to Identify Smokers: A vital sign stamp or chart sticker, or a patient questionnaire can be used to identify smokers in your practice. To make it simple, you can copy the questionnaire on the back of the QuitWorks enrollment form and give it to patients when arrive or check in.

Enrollment Forms: Fill in your practice name, address and fax number and then make copies to start using with your patients. Use the prompts on the top portion of the form to complete a brief intervention with smokers. Suggestions on how to talk with smokers are included. Then enroll patients who smoke in QuitWorks.

Brochures for Smokers: The "Think About It" brochure can be given to smokers after they have been identified (possibly at check-in) or when talking with them. It can also be used with smokers who are not yet ready to quit or ready to enroll in QuitWorks. The "Welcome Guide" brochure should be given to all smokers who fill out the enrollment sheet.

Pocket Dosing Guide for Medications: Prescriptions for FDA approved medications (including over- the- counter medications) are recommended for smokers, unless contraindicated (e.g., pregnancy). Many smokers can affordably access these medications (including over- the- counter patches and, gum) through their health insurance coverage. Be sure to make these dosing guides available to all clinicians in your practice so that patients who smoke can leave your office with a prescription.

QuitWorks Buttons: Congratulations. You are participating in a wonderful program and should let everyone know! Distribute buttons to all staff and ask them to wear them proudly and daily!

Posters: Hang these posters in the waiting room and exam rooms to encourage smokers to ask about QuitWorks and to remind providers to talk with patients about their smoking. They are available in English, Spanish and Portuguese, and are in the works for Cambodian, Cantonese, Creole, Haitian, Vietnamese and other languages.
